Care coordination can help you find and access the care you need to thrive.
You are not alone. About 1 in 5 adults in the United States will have a mental health condition in any given year. Mental health and other chronic illnesses present many challenges for children, adults, and their family members.
We recognize that taking the first step to seek help can be difficult, but we hope to make it as easy as possible for you to get the help you need. The heart of our service is building a supportive relationship with the people we serve. A Care Coordinator can help you navigate, learn about services, and get connected to them.
Individuals and families can work with a Care Coordinator who will support and assist them in navigating the sometimes confusing and complicated health care and social service systems to access the services they need to stay healthy and to improve their quality of life.
Adult Care Coordination
Any Niagara County resident 18 years of age or older, who has been diagnosed with a serious and persistent mental illness or other chronic illness, may qualify for Pinnacle Community Services’ free Adult Care Coordination services.
Our Care Coordinators will help develop an individualized care plan to help you stay healthy, out of the emergency room, and out of the hospital.
Care Coordinators will work collaboratively with you and other community service providers to help you navigate, learn about services, and get connected to the services you need to live in the community successfully and to reach your personal goals. Services are offered on an outreach basis.

Children’s Care Coordination
Children under the age of 18 who have been diagnosed with a serious mental illness, emotional disturbance or other chronic illness may qualify for free Children’s Care Coordination services.
Specially trained Children’s Care Coordinators will help develop an individualized care plan with youth and their families to help them get the mental health, medical, specialty care, and social services they need to meet their specific challenges.
Our Children’s Care Coordinators work collaboratively with community service providers to assure that the youth’s needs are met so that they can stay healthy, out of the emergency room, and out of the hospital.
